At Longmont Hearing & Tinnitus Center, we want to accommodate a variety of financing options making it easy for you to receive the care and hearing assistance products you need. We offer 0% financing with approved credit. Call our office for details.

Financial Policies

This information is provided as a general guide to patient financial obligations and Longmont Hearing & Tinnitus Center’s policies.

We recommend familiarizing yourself with your policy’s requirements, financial and otherwise. Please direct questions regarding your insurance policy or coverage to your insurance company. You can often find the member number on the back of your insurance card.


Payment for self-pay is due at the time of service.


Copays specified by your plan are collected when you arrive for your visit; your insurance company requires that you be rescheduled for another appointment if that is not possible. To avoid missing your appointment, please make sure to bring in some form of payment: credit card (Visa, MasterCard, and Discover), check or cash.

Insurance Coverage

Prior to making an appointment with our office, we strongly recommend you give your insurance company a call to ensure that your visit will be covered. Plan restrictions can include geographical limitations, or may limit you to a specific group of authorized providers.

Medicare covers hearing exams if your doctor or other health care provider orders it for medical necessity. Our office must have this Rx for medical necessity on file prior to seeing you for services. Otherwise, you will be responsible for the charges. Medicare does not cover hearing aids and exams for fitting them.

If you have a Medigap Policy (a secondary to Medicare), it is your responsibility to advise Medicare so the claim will “crossover” to that Medigap Policy. If Medicare is not aware of the secondary and the claim does not crossover during Medicare’s processing and you request that we rebill it for you; there will be a $30.00 charge for the rebilling of that claim to your secondary.

Medicaid: We no longer accept Medicaid in our office. If you have Medicare as a primary and Medicaid as a secondary, you will be responsible for any remaining balance for services not covered by your Medicare plan.

Affordable Care Plan: it is highly recommended that you double check with your plan for coverage. These plans require a written referral from your primary care physician.

Contracted Insurances

Longmont Hearing & Tinnitus Center participates with most major insurance companies. Contracted insurance companies will be billed for charges incurred during your visit.

Non-Contracted Insurances

Should you carry a non-contracted insurance policy, you will be responsible for paying any charges in full at the time of your visit. We will bill your insurance one time as a courtesy to you. You will receive any monies reimbursed by insurance. Please note that you are responsible for the full balance.

Referral Q&A

Your insurance may require that you have a referral prior to visiting a specialist. If you are not sure, please contact your insurance provider. Failure to follow your policy’s rules will result in additional out of pocket expenses. You are responsible for obtaining referrals, and will be billed the full balance should you fail to obtain and provide the necessary referrals.

Contact your primary care physician’s office

Obtain a referral from your PCP to see a specialist in your network

If the specialist refers you to another specialist, you must first obtain another referral from your PCP for the new specialist


We have a 30 day billing cycle. Any balances are due 30 days from receipt.
